This is one the ABF gets a request for on occasion and we don't have it. 

I have a scan of part of it with some kind of tab added into it... no help.

Does anyone have a copy?

I think Bill Evans put some of it in one of his books without the trio (Dummies), there has to be a scan of the original  floating around (I hope).

I have a copy of the same crappy tab on the PC here...but I think I've seen a copy of the published dots in my stacks somewhere. I can't help, I can hardly open the door to the music room there's so much other stuff stacked in there. I thought I was making headway and then we needed to use it for temp. storage. :-(
